Your first visit at Grand Dental

While you are welcome to come and spend the day with us, we understand that you probably have more exciting things to do. Below you can find some information to help you be prepared for your visit, spend as little time on paperwork as possible, as well as make sure that you can find our office.

Any previous x-rays from your last dentist (especially from the last year).

Forms

The required ones
These forms allow us to get to know you and your health. In addition, you will find the not-very-exciting, but still important information on how we protect the information that you share with us.
Request for records to be sent to Grand Dental
If necessary, this form is intended to aid you in getting pertinent dental records to Grand Dental from a previous dentist.
Email use authorization
While optional, this form allows us to communicate via email. We recommend that you fill it out no so that we do not have to be rude and legal if you try to discuss treatment of scheduled appointment over email in the future.
Consent to release information to a designated adult
Also not required, this form allows you designate another person with whom we can discuss your health information and treatment. This individual could be your spouse, a parent (for those over 18), or a child.
